Medicare late penalty calculator

If you signed up for Part B or a drug plan late, a penalty is added to your premium, usually for life. Estimate how much.

How the penalties work

  • Part B: 10% of the standard premium for each full 12 months you could have had Part B. In 2026 the standard premium is $202.90.
  • Part D: 1% of the national base premium for each month without drug coverage, rounded to the nearest 10 cents. In 2026 the base premium is $38.99.
  • Both penalties change each year with the premium, and usually last as long as you have that coverage.
  • People with Extra Help do not pay the Part D penalty. People who got Part B before 65 because of a disability have the Part B penalty removed at 65.
  • If you think the penalty is wrong, you can ask for a review. The letter about your penalty explains how.
